Overview of Services

FunctionalMatch provides informational and marketing services designed to help individuals explore potential healthcare provider options within the functional and root-cause medicine space. FunctionalMatch does not recommend, select, assign, or refer users to healthcare providers.

We are not a medical provider and do not provide medical advice, diagnosis, or treatment. FunctionalMatch is not intended for use in medical emergencies or for urgent or acute medical needs. If you have a medical emergency, call 911 or seek immediate medical attention.

Our service is intended to present users with provider options that may align with their stated preferences and inputs. Users independently evaluate and choose whether to engage with any provider.

You understand and agree that FunctionalMatch is not responsible for any medical decisions, therapies, or outcomes resulting from interactions with any provider.

Provider Network & Platform Role

Providers may apply to be included in the FunctionalMatch network. Inclusion is based on internal criteria such as areas of focus, experience, and general alignment with functional or root-cause approaches. Providers are included in our matching database after meeting our qualifications and enrolling in our platform.

FunctionalMatch operates as a marketing and discovery platform, not a referral service. Provider options presented to users are drawn from providers enrolled in the FunctionalMatch network and do not represent all healthcare providers who may offer similar services. Users are clearly informed that matches are drawn from our enrolled provider network.

We:

• Do not recommend or endorse specific providers
• Do not direct or influence patient decision-making
• Do not provide rankings based on clinical performance or outcomes
• Do not steer, nudge, or suggest that any one provider is preferable to another

Users are presented with multiple provider options within the FunctionalMatch network and make their own independent decisions. If no enrolled provider adequately meets a user's stated needs, FunctionalMatch will inform the user that no suitable match is currently available, or may present an out-of-network option with clear disclosure that the provider is not enrolled in the FunctionalMatch network.

Compensation & Transparency

Providers pay a flat monthly fee to be included in the FunctionalMatch network and receive marketing and visibility services.

This compensation:

• Is not based on patient volume, outcomes, or services provided
• Does not influence which network providers are presented to users
• Does not affect user choice or decision-making
• Is consistent and uniform across all enrolled providers

Provider inclusion and visibility are based on alignment with user inputs and platform criteria - not by payment for referrals. The matching algorithm is payment-agnostic beyond the basic filter of platform enrollment. No provider receives preferential visibility or more frequent matches by paying additional fees or offering additional compensation.

Matching Process

When users submit information, FunctionalMatch may identify providers that may align with user preferences such as:

• Location or telehealth availability
• Symptoms or conditions
• Treatment approach and treatment philosophy
• Financial alignment

This process is informational only. FunctionalMatch:

• Does not guarantee provider fit or outcomes
• Does not guarantee provider availability
• Does not schedule medical appointments without the user's explicit request and written authorization
• Does not transmit user information to providers without explicit written consent

Optional Navigation Call

FunctionalMatch offers an optional 15-minute navigation call to help users better understand provider options. This call is educational and informational only.

During a navigation call, FunctionalMatch team members:

• Will explain what functional medicine generally involves
• Will describe what each listed provider offers in terms of services or specialties
• Will answer questions about process or logistics
• Will not recommend, endorse, or direct users toward any specific provider
• Will not provide medical advice or make clinical assessments

At the start of every navigation call, the FunctionalMatch team member will state: "FunctionalMatch does not give medical advice or recommend that you select a specific doctor. Our role is to help you understand your options so you can decide."

FunctionalMatch maintains call logs after every navigation call documenting that multiple options were discussed and that no recommendation was made. FunctionalMatch team members have no financial incentive tied to a user's selection of any particular provider.

Call Recording:

Navigation calls may be recorded solely for the purpose of accurately documenting user choices and preferences. Recording will only occur with your explicit consent, obtained verbally at the start of the call. If you do not consent to recording, the call will proceed unrecorded. Recordings are stored securely and are accessible only to authorized FunctionalMatch personnel.

Scheduling Assistance & Patient Authorization

From time to time, FunctionalMatch may offer limited administrative assistance to help facilitate initial scheduling with a healthcare provider selected by a user. Any such assistance is:

• Provided solely at the user's explicit request
• Provided on behalf of the user, not on behalf of the provider
• Only initiated after the user has independently selected a provider and provided written authorization

Before FunctionalMatch facilitates any scheduling or shares user contact information with a provider, the user must confirm in writing:

"I have chosen [Provider Name]. I authorize FunctionalMatch to share my name, email, and phone number with [Provider Name]'s office to arrange an appointment on my behalf. I acknowledge that FunctionalMatch is acting on my behalf and not on behalf of the practice or provider I have selected."

This written confirmation may be completed via a click-through form, e-signature, or electronic submission. Screen-sharing assistance to complete this form is available upon request during a navigation call, provided the user directs all responses and affirms the content.

Any scheduling assistance provided by FunctionalMatch does not constitute the provision of medical care, the assumption of medical responsibilities, or the establishment of a patient-provider or patient-FunctionalMatch clinical relationship.

All medical services, scheduling decisions, and care are the sole responsibility of the healthcare provider and the user. FunctionalMatch is not responsible for scheduling errors, delays, cancellations, or miscommunications.

User Responsibilities

By using the site, you agree that:

• Information you provide is accurate and voluntary
• You are at least 18 years old
• You understand this is not medical care
• You are solely responsible for your healthcare decisions
• You will not submit detailed medical records or clinical documentation through our intake forms unless specifically requested

No Medical Advice

All content, including provider information, descriptions, and educational material, is for informational purposes only. It is not a substitute for professional medical advice, diagnosis, or treatment. Always consult a qualified healthcare provider regarding any medical condition or treatment decisions.

Data Privacy & Texas Law

FunctionalMatch handles all user information in accordance with our Privacy Policy and applicable law, including the Texas Health & Safety Code Chapter 181 and the Texas Data Privacy and Security Act. Any personal information or self-reported health or wellness information you provide to us will be used and maintained in accordance with our Privacy Policy and applicable law.

FunctionalMatch is not a HIPAA covered entity or business associate. All personal and self-reported health information is handled in accordance with applicable law, including the Texas Health & Safety Code Chapter 181.

We require all third-party vendors who access, store, or process user information to provide contractual commitments to maintain the confidentiality and security of that data. FunctionalMatch personnel who handle user information are required to complete privacy training and sign confidentiality acknowledgments.

Third-Party Services

We may use third-party services to collect, store, and process user information. By using FunctionalMatch, you acknowledge that:

• Data may be processed by third-party platforms subject to appropriate data protection agreements
• FunctionalMatch requires vendors to maintain contractual data security commitments
• FunctionalMatch is not responsible for third-party system failures or security breaches beyond our reasonable control

Use of Artificial Intelligence

FunctionalMatch may use automated tools, including artificial intelligence, to support platform functionality as described in the Privacy Policy. These tools are used solely to support administrative and informational platform functions and do not provide medical advice or make healthcare decisions. Personal identifiers are not included in AI processing unless explicitly provided for a specific action.

Intellectual Property

All content, branding, design, and materials on this site are owned by Rootmatch LLC d/b/a FunctionalMatch and may not be used without prior written consent.

Limitation of Liability

To the fullest extent permitted by law, FunctionalMatch is not liable for:

• Any healthcare decisions or outcomes
• Any interactions with providers
• Any errors, interruptions, or system issues
• Any damages arising from use of the platform

In no event shall FunctionalMatch be liable for any indirect, incidental, consequential, special, or punitive damages. Use of the service is at your own discretion and risk.

Modifications to These Terms

We may update these Terms at any time. Continued use of the platform after posting changes constitutes acceptance of those changes. We will post a "Last Updated" date at the top of these Terms.

Governing Law

These Terms are governed by the laws of the State of Texas, USA. Any legal action arising out of or relating to these Terms shall be brought exclusively in the state or federal courts located in the State of Texas.

SMS Messaging Terms

If you opt in to receive SMS communications, you agree to receive non-marketing, service-related messages from FunctionalMatch regarding your request (for example, intake confirmations, provider options updates, and reminders). Consent to receive SMS messages is not a condition of using the FunctionalMatch platform.

By providing your phone number and checking the consent box on our intake form, you expressly consent to receive automated and manual text messages from FunctionalMatch. Message frequency varies; msg & data rates may apply. Reply STOP to opt out or HELP for assistance. You may also contact us at jake@functionalmatch.com.

FunctionalMatch will not share your SMS opt-in data with third parties for marketing purposes. SMS opt-in data is not used for AI processing or shared with providers without your explicit authorization.

By providing your phone number, you consent to receive SMS messages from FunctionalMatch related to your request, including intake confirmations, provider options updates, appointment reminders, and follow-up communications. Message frequency may vary. Message and data rates may apply. Message delivery is not guaranteed and carriers are not liable for delayed or undelivered messages.
